WYOMING STATUTES, §35-22-101, et seq.
General
Any adult may execute a declaration directing the withholding or
withdrawal of life-sustaining procedures in a terminal condition.The declaration
made pursuant to this act shall be in writing, dated and signed by the
person making the declaration, or by another person in the declarant's
presence and by the declarant's expressed direction.
A declaration may be in the statutory form, but may include other provisions and does not have to appoint a person to make health care decisions.
Execution
The declaration MUST be executed in the presence of two or more
adult witnesses.
The witnesses shall not be:
(1) The person who signed the declaration on behalf of and at the direction of the person making the declaration;
(2) Related to the declarant by blood or marriage;
(3) Entitled to any portion of the estate of the declarant; or
(4) Directly financially responsible for declarant's medical care.A declaration has no effect during the pregnancy of a declarant.
Revocation
A declaration may be revoked at any time by:
(1) Being obliterated, burned, torn or otherwise destroyed or defaced in a manner indicating intention to cancel; or
(2) A written revocation of the declaration signed and dated by the declarant or person acting at the direction of the declarant; or
(3) A verbal expression of the intent to revoke the declaration, in the presence of an adult witness who signs and dates a writing confirming that the expression of intent was made.
