Anatomical Gift
Any individual eighteen years of age or older and of sound mind may make an anatomical gift for any statutorily specified purpose by execution of a document of gift.
An individual eighteen years of age or older may make an anatomical gift by will. The anatomical gift becomes effective upon the death of the testator without waiting for probate.
An anatomical gift takes effect upon the death of the donor if:
1. The donor is eighteen years of age or more;
2. The donor is less than eighteen years of age and a parent or guardian of the individual signs the statutorily prescribed authorization.

If a person less than eighteen years of age wishes to make an anatomical gift, one of the witnesses who signs the document shall be a parent or guardian of that person.
Form
A document of gift must conform substantially to the statutory form.
Revocation or Amendment of Anatomical Gift
If the will, card, or other document of gift or an executed copy thereof, has been delivered to a specified donee, the donor may amend or revoke the anatomical gift by any of the following:
1. The execution and delivery to the donee of a signed statement;
2. An oral statement made in the presence of two persons and communicated to the donee;
3. A statement during a terminal illness or injury addressed to the physician attending the donor and communicated to the donee;
4. A signed card or document found on his person or in his effects.
A donor may revoke any document of gift which has not been delivered to the donee, in any manner set out above or by destruction, cancellation, or mutilation of the document and all executed copies of it.
Any anatomical gift made by a will may be amended or revoked in the manner provided for amendment or revocation of wills or as set out above.
